WHEN DOES MY COVERAGE BEGIN?

Coverage begins on the effective date, which will be after 3 payroll deductions.

DOES THIS POLICY PAY IN ADDITION TO ANY OTHER DENTAL PLAN?

ONLY if you are covered under a spouse's policy, Kanawha will coordinate benefits with the other insurance company. In other words, if the other plan covers 80% for a filling, then Kanawha will pick up the other 20% subject to policy provisions.

WHAT HAPPENS IF I LEAVE EMPLOYMENT FROM NC DOT FOR ANY REASON?

Coverage will terminate. It is only for active NC DOT employees.

WHAT IS THE CUT-OFF AGE TO COVER CHILDREN?

Age 25, so long as they are students, single, and solely dependent upon parents for support.

HOW LONG CAN I MAINTAIN THE COVERAGE?

As long as you are an active full time employee, until age 100.

DOES THE COST GO UP WHEN I CHANGE AGE?

No, the premium only goes up IF there is a premium increase for the entire group.  The premium is guaranteed NOT to increase for two years (July 2007).

DO I HAVE TO BE PAYROLL DEDUCTED?
